DR. SRINIVAS DURSHANAPALLI [DR. D]
Dr. D is a highly experienced and compassionate dentist, dedicated to providing exceptional care to every patient. He earned his Doctor of Dental Surgery (DDS) degree from the prestigious New York University College of Dentistry, and brings over 15 years of expertise across all aspects of dentistry. Throughout his career, Dr. D has practiced in multiple states, giving him the unique ability to navigate various insurance plans and business systems while ensuring seamless patient care.
His advanced training and experience allow him to confidently manage complex cases, including oral surgery, dental implants, dentures, and full-mouth rehabilitation. Dr. D combines clinical excellence with a patient-centered approach, ensuring every treatment is tailored to the individual’s needs.
In addition to his civilian practice, Dr. D serves as a dedicated Army Dentist with a Medical Support Unit in Hobart, Indiana. Commissioned as a Captain and promoted to Major in October 2021 by the officer selection and promotion board—approved by Congress and the President—he brings his civilian expertise to the U.S. Army Dental Corps. Dr. D actively contributes to the Army Medical Recruiting Command, helping recruit and mentor qualified medical officers to serve soldiers in active and reserve capacities. He balances his military commitment with civilian practice, attending training and readiness sessions throughout the year.
When he’s not practicing dentistry or serving the Army, Dr. D enjoys mentoring young dental professionals, staying up-to-date with the latest advancements in dental technology, and giving back to the community through health education and outreach programs. His mission is to make high-quality dental care accessible, comfortable, and personalized for every patient who walks through his doors.
Dr. Chebolu
Dr. Chebolu completed her Bachelor of Dental Surgery (BDS) at NTR University of Health Sciences in India and later earned her Doctor of Dental Surgery (DDS) degree from the State University of New York at Buffalo School of Dental Medicine. Her commitment to continuing education and patient-focused care allows her to provide high-quality dentistry tailored to each individual’s needs.
Patients appreciate Dr. Chebolu’s gentle nature, attention to detail, and genuine dedication to making every dental visit a positive experience. She believes that trust and communication are the foundation of exceptional dental care.
Beyond dentistry, Dr. Chebolu enjoys a vibrant family life with her husband and their two children, Atharv and Ayra. She is passionate about classical music and trained in the Veena, a traditional Indian instrument. When she is not caring for patients, she enjoys cooking, singing, dancing, traveling, and spending meaningful time with her family.

Hayden
Meet Affinity Dental team member Hayden, a dedicated Registered Dental Assistant who brings both professionalism and positivity to every patient interaction. Hayden graduated from dental assisting school in 2024 and has been proudly serving patients with Affinity ever since.
Certified in dental radiology, coronal polishing (prophy), and pit & fissure sealants, Hayden is passionate about providing high-quality patient care while helping patients feel comfortable, informed, and at ease throughout their visit. Known for an upbeat, bubbly personality and compassionate approach, Hayden strives to create a welcoming environment where every patient feels heard, valued, and cared for.
With a strong focus on patient needs, wants, and overall experience, Hayden is committed to making each appointment as smooth, positive, and stress-free as possible while continuing to grow and excel in the dental field.
Angel
Angel is responsible for welcoming patients, scheduling appointments, assisting with check-ins and paperwork, and helping ensure each visit runs smoothly from start to finish. With a professional yet personable approach, Angel enjoys working closely with patients and takes pride in providing exceptional customer service and support to both our patients and dental team.
Known for being friendly, organized, and attentive, Angel is dedicated to helping every patient feel comfortable, valued, and cared for throughout their experience at our office.
Ashley
Meet Ashley, a dedicated Dental Assistant at Affinity Dental who is passionate about helping patients feel confident in their smiles and comfortable during every visit. Ashley began working in the dental field in 2021 and has been a valued member of the Affinity team for the past year.
Certified in prophylaxis and sealants, Ashley takes pride in providing compassionate, high-quality patient care while creating a welcoming and nonjudgmental environment where patients can feel safe, respected, and at ease. Her caring personality and gentle approach help patients feel supported throughout their dental experience.
Ashley is deeply committed to continuing her education and growth within the dental field, with future goals of becoming a dental hygienist. She looks forward to continuing to make a positive impact on the lives and smiles of her patients every day.
Kayla
Meet Kayla, our Dental Office Manager! Kayla began her dental career in 2014 after attending dental assisting school and has been dedicated to growing her knowledge and skills ever since. She holds certifications in dental radiology, cosmetic teeth whitening, coronal polishing, pit & fissure sealants, and coronal scaling, while also maintaining active BLS, HIPAA, and OSHA certifications. She is passionate about continuing education and is always looking for ways to further expand her experience in the dental field.
Having worked alongside Dr. Chebolu and Dr. Durshanapalli since before Affinity opened, Kayla continued with the team when the practice was established and has been an important part of the office ever since. Her experience spans both clinical and administrative roles, allowing her to transition seamlessly from front office responsibilities to chairside assisting. From insurance coordination, treatment planning, coding, and pre-authorizations to patient care and clinical assisting, Kayla truly wears many hats — and wears them exceptionally well.
Kayla is experienced and confident in many areas of dentistry, including general dentistry, pediatrics and special needs care, oral surgery, endodontics, periodontics, orthodontics, removables, crowns and bridges, etc. Patients frequently praise her for being knowledgeable, communicative, and compassionate, especially with children and anxious patients. Her positive attitude and bubbly personality help patients feel safe, comfortable, and cared for throughout their visit.
Known by coworkers and staff as supportive, encouraging, team-oriented, and fun to work with, Kayla consistently goes above and beyond for both patients and her team. Her leadership, dedication, and infectious energy help create the welcoming environment our office is proud to provide every day.
